3. Complete Forms Prior to Arrival
   All required forms must be completed before you arrive for your appointment. This will help streamline your check-in process.
4. Bring Your Payment
   Please bring your payment to this appointment. We accept cash, checks, Mastercard, AMEX, and Visa for payments.
5. What to Bring
   Be sure to bring a valid ID, insurance card, and any relevant medical records.
